A Practical Dental Credentialing Checklist

A workflow for organizing provider data, CAQH information, payer applications, follow-ups and recredentialing dates.

Create one verified provider record

Credentialing delays often begin with inconsistent provider information. Maintain a controlled record for legal names, practice locations, licenses, NPI details, taxonomy, education, employment history, liability coverage and payer-specific identifiers.

Confirm that the information used in CAQH and payer applications matches the supporting documents. Record who verified each item and when it was last reviewed.

Track the full application lifecycle

  • Payer research and participation decision
  • Document collection and expiration review
  • Application submission date and confirmation number
  • Missing-information requests and responses
  • Scheduled follow-up dates and contact notes
  • Approval, effective date and contract verification
  • Recredentialing and document-renewal reminders

Treat the checklist as an administrative control

Requirements vary by payer, state and provider type. A tracker improves organization but does not replace payer instructions, legal advice or professional credentialing support. Verify every submission against current official requirements.
